Kentucky Public School System
Children must be at least five years of age by October 1 of the enrollment year in order to register for kindergarten in Kentucky. Required documentation for enrollment includes a legal birth certificate, the child’s social security number, proof of residency, a Kentucky certificate of immunization, and a health exam dated within one year prior to school entry. Another health exam is required within one year prior to entry to sixth grade. Proof of an eye exam must be documented on the Kentucky Eye Exam form as well. Transferring students must comply with the state’s immunization laws within a two-week grace period. Registration is held at the school the child will be attending, although it is not necessary that the child be present at registration.
